2013 Flexible Spending and Flex Credits Deadline – March 31, 2014 
WageWorks must receive all 2013 FSA claims for reimbursement no later than March 31, 2014. The receipts must show expenses were incurred between January and December 2013. Download paper forms for FSA claims: www.myhss.org/benefits/fsa.html If you allocated flex credits to Miscellaneous Reimbursement in 2013, you have until March 31, 2014 to submit reimbursement requests to EBS. Download the EBS Miscellaneous Reimbursement Request form: myhss.org/downloads/plans/EBS_MiscReimburse_Form.pdf.
If you miss the deadline any remaining flexible spending or flex credit dollars will be forfeited. There are no exceptions. Starting with flex credit dollars you receive on or after January 1, 2014 you will no longer be required to submit reimbursement requests for miscellaneous reimbursements. This filing, however, is to receive reimbursement for 2013.

The meeting will be held at San Francisco Main Library Latino/Hispanic Rooms A&B, 100 Larkin Street at Grove. The Library is closed on Monday nights, so please plan to arrive before 6:00 p.m. If you arrive after 6:00 p.m., you can enter at the far right door of the Grove Street entrance by calling the security office at 415-557-4357. Please remember to RSPV by emailing staff@sfmea.com or calling 415-989-7244. Refreshments are available starting at 5:30pm.
HSS Has Moved 
The Health Service System (HSS), which administers employee and retiree health benefits, has moved to the 3rd floor of 1145 Market Street, San Francisco. HSS was previously located on the 2nd floor at this same address. This move will allow HSS to better serve its members with additional space and improved facilities. All HSS telephone and fax numbers will remain the same.
New HSS Wellness Center: The new HSS Wellness Center will open in the first quarter of 2014 on the ground floor of 1145 Market Street. The center will provide classes and workshops to assist employees in maintaining good health. Watch for updates on www.myhss.org .
FORM 700 - Statement of Economic Interests 
The Form 700: Statement of Economic Interests (SEI) is a form that designated officers and employees must file to disclose their financial interests annually including when assuming and leaving their office or position. Any officer or employee of the City and County of San Francisco who holds a position designated in the Conflict of Interest Code (San Francisco Campaign and Government Conduct Code, Article III, Chapter I) is required to disclose his or her financial interests.
If you have been designated as the Filing Officer to handle filing officer duties you must file the Certification of Delivery and Filing Officer Report with the Ethics Commission annually. The Certification of Delivery provides an updated list of designated employees who must file and certifies that your department has notified all filers in your department of these filing obligations as well as including the names of designated employees who have not filed their SEIs. The date by which your form must be filed for this year is April 1, 2014. 
Please Note: the Ethics Commission and the FPPC may take steps to initiate an enforcement action against a filer who is more than 30 days late in filing his or her SEI. Under state and local law, failure to file can subject the filer to a fine of up to $5,000 per violation. In addition, willful failure to file may be a misdemeanor. There is no provision in state law for extending the April 1, 2014 filing deadline. Statements filed late are subject to a late fine of $10 per day per position, up to a maximum of $100 for each day the statement is late. Fines are payable to the City and County of San Francisco. Filing officers may reduce or waive a fine on a statement filed late if the filer requests in writing to waive or reduce the fine, but only if good cause for the late filing (e.g., a documented medical reason) is shown.
